A Hospice Registered Nurse Case Manager who is responsible for the planning, coordinating, providing and documenting care for terminally ill patients from day of admission through day of discharge. Utilizing physicians’ orders and professional skills, the Hospice RN develops and implements a hospice plan that meets each patient’s specific needs and is in compliance with accrediting organization standards, federal and state regulations, reimbursement guidelines as well as the organization’s policies and procedures.
Required Skills/Abilities:
Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population.
Compassion and empathy for patients.
Attention to detail regarding a patient’s problems to analyze what they need to ease their pain and to prevent further injury.
Interpersonal skills to build positive relationships with patients, families and staff.
Communication skills to interact clearly with doctors, family members, patients and the entire team of care.
Qualifications, Education and Experience:
Graduate of an accredited school of nursing.
Current Registered Nurse licensure in the state of Utah.
Experience as an RN commensurate with one (1) of the following:
One (1) year acute care experience within the last two (2) years
One (1) year home health care experience within the last two (2) years
One (1) year community health or public health experience within the last two (2) years
One (1) year hospice experience within the last two (2) years
Current CPR certification.
Current TB test.
A valid driver’s license, reliable auto, and current auto insurance.
