The Home Health and Hospice Admissions Registered Nurse will assist the company in the admissions process.

 

  • PRN, PT
  • Must be willing to work in South Salt Lake and North Utah County

Responsibilities

 

  • Thorough, head to toe, assessment and evaluation of patient's health status at a time of admission and other timepoints, including resumption of care.
  • Initial treatment/education related to patient needs based upon referral and pertinent information gathered at time of admissions.
  • Development of a nursing plan of care, in coordination with other disciplines and providers, as ordered. Keeping the patient's and families’ goals at the forefront.
  • Assessment of patient's needs with regards to therapy, social work, home health aides and coordination/initiation of care of those disciplines -
  • Responsible for notification to provider of initiation of home health services and communication of situation as assessed including development of orders for other services, treatments
  • Responsible for timely documentation and submission of agency paperwork to Medicare and Valeo Home Health's standards including completion of OASIS documentation and associated orders.
  • Ensure compassionate care provided reflecting the mission and values of Valeo Home Health.
  • Understanding of home health regulations with regards to skilled and necessary care and homebound status.
  • Ensure compliance of admission nurse activities with the standards of quality healthcare.
  • Attends in-services and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.

Qualifications

Required Skills/Abilities:

  • Demonstrate knowledge and skills necessary to provide care to the geriatric and adult population.
  • Compassion and empathy for patients.
  • Attention to detail regarding a patient’s problems to analyze what they need to ease their pain and to prevent further injury.
  • Interpersonal skills to build positive relationships with patients, families and staff.
  • Communication skills to interact clearly with doctors, family members, patients and the entire team of care.

Qualifications, Education and Experience:

  • Graduate of an accredited school of nursing.
  • Current state licensure as a Registered Nurse.
  • Experience as an RN commensurate with one of the following:
  • One (1) year acute care experience within the last two (2) years;
  • One (1) year home healthcare experience within the last two (2) years;
  • One (1) year community health or public health experience within the last two (2) years;
  • One (1) year outpatient facility experience within the last two (2) years;
  • Credentials in specialty areas, i.e., phlebotomy, IV, ET.
  • Current CPR card.
  • Current TB test.
  • Valid state driver’s license and reliable automobile.
  • Proof of current automobile insurance.
