Assists physical therapists in delivering comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative physical therapy as prescribed by a physician.

Responsibilities

  • Ensures that changes in patients’ clinical status are communicated to oversight physical therapists and other healthcare team members and HHA office staff verbally and in writing in a timely manner.
  • Ensures any need for additional/new physician orders are communicated to oversight physical therapists, other healthcare team members and HHA staff in a timely, accurate, and thorough manner.
  • Coordinates care planning and delivery in coordination with the oversight physical therapist with the physician, patient and family/caregivers, other healthcare team members and HHA staff to facilitate optimal patient outcomes.
  • Educates patients, their families/caregivers and other staff as appropriate, on appropriate physical therapy modalities.
  • Delivers physical therapy modalities appropriate to the patient’s clinical status and needs, and in accordance with the standard of care for patient’s age developed by the Physical Therapist.
  • Coordinates need for alterations in plan of care and physical therapy modalities with oversight of physical therapist based upon changes noted in clinical status of the patient.
  • Coordinates the need for durable medical equipment as well as other adaptive equipment needs with oversight of Physical Therapist.
  • Implements appropriate home exercise programs tailored to the patient’s clinical status, needs and age based upon assessment by oversight Physical Therapist.
  • Addresses patient’s pain status and coordinates with oversight of physical therapist appropriate means to treat.
  • Utilizes appropriate pain management modalities that include, but are not limited to, physical therapy modalities in coordination with oversight Physical Therapist.
  • Educates the patient and/or caregiver in effective pain management modalities in coordination with oversight Physical Therapist.
  • Provides physical therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors in coordination with oversight Physical Therapist.
  • Identifies risk of injuries or threats to life and health in the patient’s environment, i.e., environmental/safety/mobility risks and implements appropriate interventions to reduce those risks in coordination with oversight Physical Therapist.
  • Addresses and supports cultural practices providing such practices do not harm the patient or others, and do not interfere with the planned course of physical therapy in coordination with oversight Physical Therapist.
  • Performs transfer and discharge planning functions in accordance with HHA policy and applicable laws, regulations and standards in coordination with oversight Physical Therapist.
  • Complete patient documentation on time.
  • Attends in-service and staff meetings as needed.
  • Completes assigned training on time.
  • Completes annual education requirements.
  • Maintains patient confidentiality at all times.
  • Performs other duties as assigned.

Qualifications

  • Current valid State Physical Therapy Assistant license.
  • Graduate of an accredited physical therapy assistant curriculum/school.
  • For Medicare certified agencies:
  • Graduate of a physical therapy assistant curriculum approved by: The American Physical Therapy Association, or The Committee on Allied Health Education and Accreditation of the American Medical Association, or The Council on Medical Education of the American Medical Association and the American Physical Therapy Association; OR, Two (2) years of work experience.
  • Prior home health experience, preferred.
  • Valid state driver’s license and reliable automobile.
  • Proof of current automobile insurance.
  • Current TB test.
  • Criminal background check.
  • Current CPR card.
